Mark your calendars! Dahl Memorial Clinic will be providing sports physicals on July 18th & 19th for a cost of $30.

Carla Dowdy, PA-C will be doing girls sports physicals on Tuesday July 18th from 9:00 a.m. to 4:00 p.m. Dale Diede, PA-C will be doing boys sports physicals on Wednesday July 19th from 9:00 a.m. to 4:00 p.m.

Forms are available online at http://www.ekalaka.net or in the front entrance at Dahl Memorial. Student athletes are asked to have the form filled out and signed by their parent/guardian prior to bringing it to the clinic. The form must be signed by a parent/guardian before a physical can be performed.
