Junior high and high school sports physicals

Mark those calendars. Sports physicals for those entering fifth through twelfth grades will be held on June 19 and 26 for only $25. The fee is due at time of service and can be payed with cash, a check or a credit card. Girls’ sports physicals are scheduled for June 19 from 9 a.m. to 4 p.m. and boys’ sports physicals are on June 26 from 9 a.m. to 4 p.m.

MHSA Pre-Participation Physical Forms are available online at http://www.ekalaka.net (Athletics/Athletic Forms/ Pre-Physical Exam Form) or in the front entrance at Dahl Memorial. Be sure to have the form filled out and signed by a parent/guardian prior to bringing it to the clinic. An exam will not be performed if the form is not signed by a parent/guardian.

The sports physical process is changing slightly this year. All sports physicals done on June 19 or 26 will not be billed to insurance, and the payment of $25 will be due at the time of service. Anyone who needs a physical but will not able to make it on June 19 or 26 should call Dahl Memorial Clinic at 775-8738 to schedule an appointment. Physicals scheduled via appointment will be charged at the regular clinic visit rate and billed to insurance.
