Sports physicals make-up day, July 24

A make-up day for sports physicals for all local athletes in grades 5-12 will be held on Wednesday, July 24 from 9 a.m. to 4 p.m. at Dahl Memorial Clinic. Physicals cost only $25 and payment is due at the time of service.

MHSA pre-participation physical forms are available online at www.ekalaka.net by clicking Athletics/Athletic Forms/Pre-Physical Exam Form, or can be found in the front entrance at Dahl Memorial. Be sure to have the form filled out and signed by a parent/guardian prior to bringing it to the clinic. An exam will not be performed if the form is not signed.
